1976年Sporn等[1]提出“化学预防（chemoprevention）”的概念，指用化学药物和饮食来干预肿瘤的发生过程，从而降低肿瘤的发生危险。化学预防对乳腺癌、结肠癌和前列腺癌等肿瘤的作用已有很多的研究，并取得了一些可喜的进展，但在肺癌领域进展甚微。1996年在《新英格兰医学杂志》上发表的b-胡萝卜素联合维生素A用于肺癌化学预防的报道[2]，让人大失所望。曾经认为β-胡萝卜素和维生素A具有“抗癌”作用，这项研究发现用β-胡萝卜素和维生素A长期干预肺癌高危人群，干预组患肺癌的相对风险显著高于安慰剂对照组，相对风险为1.28（95％可信区间1.04~1.57，P=0.02），肺癌发生的风险不降反升。最新的Meta分析资料也显示β-胡萝卜素增加吸烟者患肺癌的风险[3]。近年来肺癌的化学预防研究一直处于“低谷”之中。
